Information collected
The discovery form is not yet accepting submissions. While intake is closed, no information is collected through it.
When intake opens, the form will collect professional contact information, practice and workflow characteristics, consent choices, and controlled referral attribution. Email you send us carries whatever you choose to include. Do not submit patient, case, or clinical information by either route.
Use and retention
Authorized operators use responses for workflow research, pilot qualification when separately requested, and future-offer research when contact permission is given.
